About the PCN

South Sefton Primary Care Network brings together nineteen GP practices to deliver a wider range of joined-up services for patients across the South Sefton region.

Working together for South Sefton

Primary Care Networks are groups of GP practices working with community, mental health, social care and voluntary services to offer joined-up care for their patients.

Our network delivers enhanced services that no single practice could offer alone, from urgent same-day access and proactive home visits to specialist clinics and a wider team of clinicians working alongside your GP.
