Based on the information you have supplied, it is likely you are eligible. Practices will be identifying patients based on information held in medical records, and will contact you as soon as we have permission to start consulting with patients who are eligible. We can't do this yet, as we are waiting for the Cheshire and Merseyside Integrated Care Board, who organise healthcare in our area, to finalise the service specification that we will be required to work to.
You don't appear to be eligible yet - if you have 4 / 5 conditions and believe your BMI is over 40, you may need to update your practice. Otherwise, here is the timetable for the expansion of the programme, and when you are likely to become eligible for NHS treatment.
