ACE’s stands for ‘adverse childhood experiences’, which are stressful or traumatic experiences, which can include any form of abuse, neglect, witnessing domestic violence or growing up with substance abuse, mental illness or parental conflict.

Associate psychological practitioners

The APP’s provide a psychologically informed early intervention for patients with low level psychological need and up to 6 sessions of psychoeducational intervention. Rather than a formal psychological therapy or counselling service, we provide education around low level psychological problems and practical strategies that can support and help people manage these.

The Enhanced Health at Home Team works with your GP Practice. If you are aged 65 and over, have a long term condition or frailty and have had a recent hospital stay or A&E attendance, a Care Coordinator could support you with condition management and independence.
